Whether you choose to buy an insured or self-funded health plan, PCHP offers a variety of cost-effective plan-design choices.

Preferred Provider Option (PPO) Plans

PPO plan designs encourage participants to see network providers by offering them a higher level of benefits than if they see an out-of-network provider.

Point of Service (POS) Plans

POS plans are similar to PPO plans, but they require covered persons to select an in-network personal care physician.  Under a POS plan, participants can seek care outside of the network and still have coverage.  In-network care is covered at higher benefit levels than out-of-network care.

Exclusive Provider Option (EPO) Plans

EPO plans are similar to POS plans, but offer lower out-of-network benefits.

Consumer-Directed Plans

New consumer-directed health plans provide an incentive for employees to take responsibility for their health care by combining a high-deductible health plan with individual tax-advantaged health savings accounts (HSAs) or employer-funded health reimbursement arrangements (HRAs).

Wellness Incentive Plans

A wellness incentive plan encourages employees to take better care of their health and thereby reduces the demand for health care resulting from illness.  These plans are gaining popularity among employers because of the positive impact on the health of their employees.
