The term "lifetime" refers to the period of time a person is actually covered under a plan, including any amendment or restatement of the plan.  Lifetime does not suggest coverage will be granted for claims that are incurred before the effective date of an individual or after the termination date of an individual or of the plan.

Lifetime Maximum Benefit
A lifetime maximum benefit is the maximum amount a health plan will pay in benefits to a covered person during that individual's lifetime.  Once an individual has exhausted his or her lifetime maximum benefit under the plan, the plan will exclude any additional charges.
